Registered Nurse
  • Requires minimum 2 years nursing experience in a Med/Surg, Home health/Hospice or ICU setting.
  • Assumes responsibility for coordination of care with the interdisciplinary team, patient/family, DME, Pharmacy, referring Physician/agency etc.
  • Must have good interpersonal and communication skills with ability to problem-solve.
  • Ability to identify physical, psycho-social, and environmental needs and to implement /develop/document the plan of care to ensure quality and continuity of care with appropriate documentation.
  • Must be willing to perform on-call responsibilities and services to patients/families as assigned.
  • Must be a team player.
Medical Social Worker
  • Prefer LCSW. Bachelor’s degree must be supervised by Master’s of Social work.
  • Minimum of one years experience in Home Health or Hospice.
  • Primary function is to provide coordinated care in the home to patients of all age groups and perform psychosocial assessments, analysis, counseling and referrals to meet the needs of the patient/family.
Licensed Vocational Nurse
  • Must have minimum 2 yrs nursing experience.
  • Primary function is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence, and coordinate care with the interdisciplinary team, patient/family and referring physician/agency.
Chaplain
  • Hospice experience preferred.
  • Primary function is to provide spiritual care in the home to patients of all age groups and perform spiritual assessments, analysis, counseling and referrals to meet the needs of the patient/family.
Hospice Aide
  • Must have 1 year of experience.
  • Primary function is to provide direct patient care to patient under direction of the RN and according to the Aide Plan of Care in their place of residence. To assist in providing a safe and clean environment, to work cooperatively with patient and family and share observations and problems with the supervisor.
